ACERS Respiratory service


A message from Les to fellow patients of the ACERS Respiratory service about the flu jab.

We provide integrated chronic respiratory disease management across both primary and secondary care, to patients diagnosed and living with chronic respiratory disease, including Chronic Obstructive Pulmonary Disease (COPD), Asthma, Bronchiectasis and Interstitial lung disease (ILD). 

Please be aware ACERs Respiratory cannot provide a diagnostic service for COPD and Asthma, if the diagnosis is uncertain please refer to the Respiratory Medicine Department

Service includes:

  • Acute Respiratory Infection Virtual Ward
  • Admission avoidance and Early Supported Discharge
  • Respiratory Diagnostic Hublets
  • Respiratory Case Management
  • Specialist Asthma Service – Primary and Secondary Care
  • Cardiorespiratory Rehabilitation – including Pulmonary, Heart Failure and Cardiac Rehabilitation
  • Home Oxygen Assessment and Review Service
  • Integrated Psychology Service
  • Respiratory Specialist Physiotherapy Outpatient Service

Referral Criteria

  • Over 18 years old (6 years and over for the diagnostic hublet)
  • Registered with a GP in City and Hackney
  • Lives in City and Hackney
  • Confirmed Diagnosis of a Chronic Respiratory Disease
