Change or cancel your appointment
Your GP or consultant has requested an appointment for you, so it is essential you attend.
If you cannot make your appointment for any reason, let us know as soon as possible so we can give the time to someone else.
Fill in the form below or call the number on your appointment letter to tell us if you:
- cannot make your appointment and would like a new date: we will contact you to provide an alternative appointment.
- would like to cancel your appointment entirely: we will send a letter to you and your GP to say you have been discharged from our care.
