Welcome to the IT Enabler Programme web page.

The IT Enabler programme is a programme of work that helps join up provider organisations digitally across City & Hackney in the journey toward integrated care.

Key projects within the IT Enabler programme include east London Patient Record (HIE), the Urgent Care Plan (UCP) and the emerging work for the Patient Held Record (PHR), all involving provider organisations in City & Hackney working in partnership to support the patient journey.
