Patient Safety Incident Response Framework (PSIRF)
What is PSIRF?
The Patient Safety Incident Response Framework (PSIRF) is the approach we use to respond to patient safety incidents, focusing on transparency, supporting those affected, and continuous learning to improve healthcare safety.
The framework has four key aims:
- Compassionate engagement with those involved.
- Using system-based methods to learn from incidents.
- Responding to incidents in a thoughtful and balanced way.
- Offering supportive oversight to enhance learning and improvement.

Along with our PSIRF policy, a PSIRF plan has been developed which outlines how we will respond to patient safety incidents to maximize our learning and improvement. It was developed based on our incident profile, ongoing improvement priorities, resource availability, and the priorities identified by our stakeholders.
