
One year of PSIRF: 3rd - 7th February 2025
The first week of February marks one year since the Patient Safety Incident Response Framework (PSIRF) was introduced across the Trust. PSIRF represents a huge shift in how we respond to and learn from patient safety events, and to recognise the journey we have all been on, we are planning some events from Monday 3rd – Friday 7th February. We hope to talk to people about PSIRF, reflect on how it has felt for staff and patients involved (the good bits and the bad bits), and share ideas for what comes next.
The key events during the week are:
Wednesday 5th February – 11:30 – 12:45 – Costeloe Lecture Theatre, Education Centre: One year of PSIRF through the eyes of those involved
You are all warmly invited to hear from a panel of colleagues from across the Trust discussing how PSIRF has felt from the perspective of those who have been involved in incidents, learning responses and designing improvements.

Monday 3rd – Friday 7th February: PSIRF information stall, come and meet the team!
Throughout the week we will be visiting a number of Trust locations to speak to staff, share information and resources (including details on training available), and answer any questions people may have.
We will be hosting a stall in the following areas:
- Monday 3rd February, 9am – 12noon: Main reception, Homerton University Hospital
- Tuesday 4th February, 9am – 12noon: St Leonards Hospital
- Wednesday 5th February, 1-2pm: Chatters Restaurant, Homerton University Hospital
- Thursday 6th February, 9-11am: Hackney ARK
- Friday 7th February, 9-11am: Kenworthy Road
